Overview:
A community healthcare facility is seeking a Director of Case Management to oversee clinical and financial operations within the Care Management system. This leadership role provides managerial and educational direction in the areas of care management operations, resource management, and patient flow throughout the continuum. The position offers a competitive salary of $125,000 – $130,000 along with a Bonus Incentive Plan.

Key Responsibilities:

• Responsible for the clinical and financial operations of the Care Management system.
• Provide leadership in care management operations, resource management, and ensuring efficient patient flow through the continuum.
• Manage a team of 5 direct reports, including 3 full-time Case Managers (CM) and 2 PRN Case Managers, with a mix of RN/SW.
• Report directly to the CEO.
• Ensure effective managerial and educational direction for Case Management staff.
• Director of Case Management does not carry a patient case load.

Qualifications:

Education:

• Open to licensed RN or SW/MSW.

Experience:

• 2+ years of acute care hospital leadership in case management.
• Professional experience with a focus on strong customer service.
• Ability to manage a 12 average case load.

Certifications:

• Must be willing to acquire Certification in Case Management within the first year of employment if not already a Certified Case Manager.
